1. An ACCESS REQUEST FORM (ARF) is the standard expression for a type of form connected to the National Disability Insurance Scheme (NDIS), or the administration of the scheme by the National Disability Insurance Authority (NDIA). People fill out the form so that NDIA can identify if a person is eligible to become a participant.
